EMERGENCY INFORMATION AND PROCEDURES

 

An emergency information card must be on file for each student. When there is a change of address/phone number or a change in the child's medical history, the parents should inform the school office immediately so that the necessary changes may be made on the information card.

 

In the event that the parents go away and leave their child in the care of someone else, the office must be notified in writing. This information must include the name(s) of the person(s) having temporary responsibility, their home and business telephone numbers and a permission note indicating that they take full responsibility for any action to be taken in case of an emergency.

 

 

 

CHILDREN'S HEALTH

        Children who must take medication during the school day:

If any child contracts a communicable disease, please notify the school office, so we may, in turn, notify parents of other children in the same room.

If a child becomes seriously ill or is injured at school, parents will be notified immediately. Parents must pick up the child at the school office or make arrangements for transportation.

If a child is hospitalized or has a lengthy illness, parents may take advantage of home-bound instruction, which is provided by the school district in which you live.
