WHEN DISTRIBUTING THE 2009 H1N1 INFLUENZA VACCINATION
PACKAGES***
We are now handing out very important papers to every student in our school who resides in the
East Stroudsburg Area School District. These papers explain the FREE H1N1 (or Swine) flu
vaccinations that their school district is offering to them next week. Every student in the East
Stroudsburg Area School District must give these papers to their parent, guardian, or other adult
at home tonight.
If you live in the East Stroudsburg Area School District. and your parent wants you to get a
FREE vaccine, they must complete the permission form on the last page and bring it with them
to one of the clinics. Do not return the permission form to the school, but take it with you when
you go to get your vaccine. Clinics will be held at the J. T. Lambert Intermediate School next
Wednesday, November 18th and the following Saturday, November21st.
Students cannot get a vaccine unless their parent or another responsible adult is with them. So,
please make sure you give these papers to your parents as soon as you get home.
