IMMUNIZATION REQUIREMENTS

 

The Pennsylvania Department of Health regulations state that: no student may be admitted to school without written proof of having received the following immunizations:

§4 doses of Diptheria, Pertussis, Tetanus (DPT) or DiptheriafTetanus (Dt, TD) with one dose given on or after the fourth birthday.

§3 doses of Trivalent Oral Polio Vaccine.

§1 dose of Measles, Mumps, Rubella Vaccine (MMR) given after 12 months of age.

§2nd dose of Measles Vaccine given preferably as an MMR.

§3 doses of Hepatitis B Vaccine (HBV).

§1 dose of Varivax Vaccine or Written proof of having had chickenpox.

 

All written proof of immunization must include the dates that the immunizations were received.
